What is Form?
Form is an iPhone application that produces a structural rating from a single portrait. The output includes an overall score, a PSL rank, a component analysis, and ranked improvement priorities.
How is my score calculated?
Form combines structural detection with a calibrated rating model. The same score always maps to the same rank; score and rank are produced in lockstep and cannot contradict each other.
Why did I get a different score on a second scan?
Small variations are expected. Lighting, angle, expression, and image quality influence what the model sees. For the most consistent reading, use front-facing, neutrally lit portraits with a relaxed expression.
Is my photo private?
Your photo is stored locally on your device. A temporary copy is sent to our AI processing vendor only to generate the analysis, and is not retained after processing. Read the full Privacy Policy for details.
How do I cancel my subscription?
Open Settings → [your name] → Subscriptions on your iPhone and select Form. Cancellation takes effect at the end of your current billing period. You retain access until then.
How do I restore my subscription on a new device?
Open Form, reach the paywall, and tap Restore Purchases. Sign in with the Apple ID used for the original subscription. Your entitlement will be refreshed automatically.
I would like a refund.
Refund requests are handled by Apple. Visit reportaproblem.apple.com, sign in, and select the Form purchase. Form cannot process refunds directly.
Can I delete a past scan?
Yes. In Form, open your session archive, select the scan, and remove it. Uninstalling the app deletes all local history.
What if the AI analysis fails?
If the AI step is unavailable, Form falls back to its structural-only rating so you still receive a report. When the AI is available again, new scans will include its output.
